Good Faith Estimate

If you are uninsured or choose not to use your health insurance (self‑pay), you have the right to receive a Good Faith Estimate* of the expected cost of your chiropractic care before services are provided. This estimate shows the expected charges for your scheduled items or services and related care.

You may request a Good Faith Estimate before scheduling your appointment or at any time. If you schedule care at least three business days in advance, we will provide the estimate no later than one business day after scheduling. If care is scheduled 10 or more business days in advance, we will provide the estimate no later than three business days after scheduling. If you request the estimate, it will be provided within three business days of your request.

Make sure to keep a copy of your Good Faith Estimate. If you receive a bill that is $400 or more higher than your Good Faith Estimate, you may contact us to review the difference and, if necessary, initiate a dispute with the provider through the federal process.
